Low FODMAP Diet: Complete Beginner Guide
What FODMAPs are
FODMAP stands for Fermentable Oligosaccharides, Disaccharides, Monosaccharides, And Polyols — short-chain carbohydrates that are poorly absorbed in the small intestine. They ferment in the colon, producing gas and drawing in water; in sensitive individuals this causes bloating, cramping, diarrhea, or constipation.
Evidence base
The protocol was developed at Monash University. Randomized trials show ~70% of IBS patients experience significant symptom reduction following structured low-FODMAP elimination + reintroduction (Halmos et al., Gastroenterology 2014).
This is not a long-term diet. Continuous strict low-FODMAP is associated with reduced microbial diversity. The goal is to identify your triggers, then liberalize.
The 3-phase protocol
- Elimination (2–6 weeks). All high-FODMAP foods removed. Most symptom-responders see improvement in 1–2 weeks.
- Reintroduction (6–8 weeks). One FODMAP subgroup at a time, every 3 days. Identify which subgroups trigger and at what dose.
- Personalization (ongoing). Long-term diet allows tolerated foods, restricts only personal triggers.
Foods to eat and avoid (selected)
Low (eat freely): Rice, oats, quinoa, eggs, lean meat, fish, lactose-free milk, hard cheeses, blueberries, strawberries, carrots, spinach, cucumber, bell peppers, bananas (unripe).
High (avoid in elimination): Wheat, rye, onion, garlic, apples, pears, mango, watermelon, milk, soft cheese, ice cream, kidney beans, honey, high-fructose corn syrup.

FAQ
Should I do this without a dietitian?
Strict elimination for 2+ months without guidance risks nutritional gaps and microbiome disruption. Work with a Monash-trained dietitian if possible.
How do I know it's working?
Most responders see ≥50% reduction in primary IBS symptoms (bloating, abdominal pain, altered bowel habits) within 4 weeks of strict elimination.
